Elderly, Disabled to Get Medical Care Option
Health System chapters is to create a program of All-Inclusive Care for the elderly, following a model in use at the national level.
This is a Medicare / Medicaid funding to coordinate and provide all necessary health care for this high risk group. At least 75 organizations PACE are active at national level, in places including Florida, Pinellas, Lee, Charlotte and Miami-Dade counties.
Preventive care, primary care, other medical care, hospitalization and, if necessary, long-term care are covered, although the goal is to keep these patients — who have medical conditions that qualify them for nursing homes — living in the community.
"This is very much prevention-based, focused on keeping people out of the hospital," said Andy Lutton, chief operating officer of Chapters, formerly HPC Healthcare.
Legislators in Florida, after six years to allow PACE programs in Florida are more sensitive as they see its potential to reduce care costs by delaying or preventing admissions to nursing home, said Samira K . Beckwith, president of Health Services and president of Hope a Florida PACE Association.
Nursing home costs are a significant portion of Medicaid spending in Florida editing.
PACE is a natural extension for the chapters, best known for providing palliative care through the Good Shepherd and LifePath Hospice, Lutton said. Chapters also palliative care at Winter Haven Hospital and several hospitals in Tampa.
"By expanding our continuum of care, we will be able to help more people in need," said Lutton. "We are still focusing on the frail elderly ... to have their health needs met in a very family-centered way. "
The chapter concludes the application process, who said Lutton is long and complex to begin enrolling patients in Hillsborough County this fall. It will have 100 seats there.
Expansion into Polk County is expected in summer 2012. This will provide 150 places, with the Highlands and Hardee counties included.
